St. Agnes Academy-St. Dominic School is proud of our Emergency Management Plan.  We take great care to provide a safe environment for your children, and one component of that is the information you provide to us in the Student Emergency Information Form.  We ask that you complete a new form for each child at the start of the new school year so that all information is as current as possible.  This is the form we will draw information from in the case of any illness, accident, or emergency at school.  Please take your time and complete the form thoroughly so that we may provide the best care for your child at all times.  Blank forms will be available at registration and at the school offices throughout the school year.
